At the port forwarding creation screen, it lists the following criteria:
Application Name
Protocol Type
Port Start
Port End
Port Map Start
Port Map End
in regards to protocol type, I can choose:
TCP
UDP
TCP,UDP
Do I only choose UDP?
What do I enter in the Port Map Start and Port Map End boxes?
I've read that some ISPs block certain ports to discourage people from hosting webservers etc. Could it be a factor?

Try this:
Application Name = whatever you want (LOTR Conquest or something)
Protocol Type = UDP
Port Start = 2350
Port End = 2360
Port Map Start = 2350
Port Map End = 2360
Repeat this for the port you use for Tunngle (for a single port, just make the "port start" and "port end" the same). The most important thing you didn't mention is the option to pick which PC on the network this is going to be applied to. You'll either get a drop-down box which lets you select your PC by name, or you'll have to run 'ipconfig' (or find your PC on the DHCP page of your router's configuration webpage) to get your LAN IP address (192.168.0.something for D-Link routers, if I remember correctly). I haven't tested this out personally because my router is fine with upnp, so let me know how it goes.
ALSO you will need to repeat the same thing for Windows Firewall though that should be a simple matter of adding the program to a list of programs "allowed" to access the internet, and you shouldn't have to enter the specific port numbers. You'll have to do that for Conquest and for Tunngle.
ISP's are likely to block common applications, particularly web/FTP/mail servers but I wouldn't worry about that for your purposes. They use different ports.
